GentlemEiV,—Upon your admitting into tlic Dundee Lunatic Asylum as a patient, I hereby bind and oblige myself to pay the board fixed by you for the said ; to remove when required to do so by you; to bury in case of dcatli; to kwp up a proper stock of necessaries, as mentioned in one of the foregoing lists, and to renew them when destroyed or ■worn out; and, if tlie foresaid necessaries, or any of them, are not furnished when required by notice, in writing, from the Secretary of the Asylum, the Directors or their Committee may, in ten days after such notice, order such necessaries to be provided at my expense, wliich T hereby bind and oblige myself to pay; and, generally, to fulfil all tlie obligations required by the Regulations of the Institution. I am. Gentlemen, your obedient servant, To the Directors of the Dundee Royal Lunatic Asylum, Dundee. N.B.—Persons applying for the admission of patients are particularly requested to have the foregoing certificates and obligations correctly dated. The medical certificate must be done in duplicate. Women in a state of pregnancy, and patients labouring under the delirium of typhus fever, &c., are inadmissible. All the Certificates to he sent under cover to the Surgeon-Superintendent.
