    protection in over 80% of recipients; in those not fully protected the disease is usually less severe. The two large epidemics which followed the reduction in vaccine acceptance are additional evidence of the effectiveness of whooping cough vaccine in the prevention of disease. In Regions with particularly low vaccine acceptance rates, whooping cough notifications in 1977—79 were significantly higher than those in Regions where acceptance was higher than the national average. 3.2 Vaccine 3.2.1 Whooping cough vaccine is a suspension of killed Bordetella pertussis organisms with an estimated potency of not less than 4 International Units in each 0.5ml of vaccine. The vaccine is usually given as a triple vaccine combined with diphtheria and tetanus vaccines, with an adjuvant such as aluminium hydroxide (DTPer/Vac/ Ads, Trivax Ads). It is also available without the adjuvant as plain DTPer/Vac, and as a monovalent pertussis vaccine. The adsorbed vaccine should be used as it is more immunogenic and causes fewer systemic reactions, especially fever. 3.2.2 Adsorbed diphtheria/tetanus/pertussis vaccine (DTPer/Vac/Ads): one 0.5ml dose consists of a mixture in isotonic saline of diphtheria toxoid and tetanus toxoid adsorbed on to aluminium hydroxide gel, together with not more than 20,000 million Bordetella pertussis organisms. The potency of the diphtheria component is not less than 30 IU; that of the tetanus component not less than 40 IU and that of the whooping cough component not less than an estimated 4 IU. Thiomer- sal is added as a preservative to a final concentration of 0.01 per cent. 3.2.3 Monovalent pertussis vaccine: one 0.5ml dose contains not more than 20,000 million Bordetella pertussis organisms. Thiomersal is added as a preservative to a final concentration of 0.01 per cent. The vaccines should be stored between 2—8°C, but not frozen. 0.5ml should be given by deep subcutaneous or intramuscular injection. 3.3 Recommendations 3.3.1 Adsorbed whooping cough vaccine as a component of the primary
