[Owner or Occupier, if any such there be,] and to the Town Council, <$•<?., or to the Guardians of the Poor of the Union or Parish [as the Case may be,] and to their Servants, and to all other Persons whom this order may concern. County of 1 Whereas on the Day of [or Borough, $c. of L last Complaint was made before B.C. and ] to wit. ) E.F., Two of Her Majesty’s Justices of the Peace acting in and for the County of [or Borough, of as the case may be,] by the Town Council, #c., [or by the Guardians of the Poor of the Union, as the Case may be,] that the Dwelling House occupied by the said X.Y. situate in Street in the Parish of in the said County of [describing the Premises] was in a filthy and unwholesome State [or that there was an Accumulation of offensive or noxious Matter, Re- fuse, Dung, and Offal on a certain Piece of Land near the King’s Head Inn situate, #c., or that there was a foul and offensive Drain, Privy, or Cesspool in certain Premises occu- pied by one Y.Z. situate, fyc., [as the Case may be],] and’the Certificate in Writing under the Hands of A.B. and C.D. of being Two duly qualified Medical Prac- titioners, addressed to the Town Council, §c., [or to the Guardians of the Poor of the Union, as the Case may be,] certifying that the same was likely to be prejudicial to the Health of the Occupiers, or of the Persons whose Habitations are in the Neighbourhood thereof, having been produced before the said Justices at the Time of the making of the said Complaint: And whereas the said X.Y. the Owner [or Occupier] of the said Dwelling House [or of the said Piece of Land, or of the said Premises] herein- before described, has this Day appeared before us, B.C. and E.F., Two of Her Majesty’s Justices of the Peace acting in and for the said County [or Borough, $-e.] in pursuance of a Summons duly served upon him in that Behalf, to answer the Matter of the said Complaint.
