![Write down EVERYTHING your child has to eat or drink (NOT ONLY the food and drink taken at main meals). Include any tonics, vitamin preparations, or medicines that your child may take. Begin a NEW double page for each day. If necessary, more than one double page can be used for one day’s record. Write in the day of the week and the date at the top of each page. Write in, in COLUMN 1, the time at which the meal, snack or item of food or drink was eaten or drunk. Write descriptions of the food or drink in COLUMN 2 and enter their weights in COLUMNS 3 and 4, using a new line for each item of food or drink. Fill in every day, in the space provided at the bottom of the right hand page, the amount of milk you bought that day and the time your child went to bed at night. The big numbers on the scale are ounces and the little numbers between them are one-sixth ounces. First write in the big numbers in COLUMN 3 and then write in the small number in COLUMN 4. If the item weighs less than one ounce, write in ““O” in COLUMN 3. Some items (e.g. an apple, a biscuit, a slice of bread) can be weighed directly on the scale.
