![21 Notification of Births Act. [Form of Notification.] Reference (Urban District Council of Barnet, NOTIFICATION OF BIRTHS ACT, 1907. To the Medical Officer of Health. Name of parents Address Date and time of birth, being alive or dead If parent not to be visited, please state—No visit required State whether Boy or Girl If Midwife is in sole charge of Patient, her Name and Address is requested Name and Address of Person sending card 1. Notification of birth must be sent to the Medical Officer of Health within 36 hours after birth, under a penalty not exceeding 20/- in case of default. 2. The duty of notifying falls firstly on the father, but if not living at home at the time, secondly on any person in attendance at the time of, or within 6 hours after the birth. 3.
