The applicant for whom work is forbidden may be told the doctor recommends rest and he may be advised to return when the doctor will approve work. E. Physical Capacities Report for Use in Securing Information from Physicians The physical capacities report shown on the following pages has been designed for the use of doctors, either private phy- sicians, physicians on the staff of hos- pitals or the State vocational] rehabilita- tion agencies or other agencies, or any other physicians from whom physical capacities information is requested. The form is made up of two parts: one part which is in effect a letter to the doc- tor |and a separate sheet (printed front and back) which is the capacities report itself. The information on the request to the physician should be filled in by the local office. The entries are self- explanatory. This part of the form contains the ap- plicant’s waiver and can be kept by the physician: The physical capacities re- pert will be returned by the doctor to the local office. The applicant’s name and address should be filled in by the local office on the physical capacities report form.
