To of Take notice that the Insurance Commissioners have under consideration the question of holding an Inquiry with respect to the matters appearing in the subjoined statement, for the purpose of ascertaining whether your continuance on the panel of medical practitioners for the County [County Borough] of would be prejudicial to the efficiency of the medical service of insured persons. You may, if you so desire, inform the Commissioners by statement in writing, addressed to me, within seven days after receipt of this notice, whether you admit or dispute in whole or in part the truth of the matter appearing in the said statement. If the Insurance Commissioners decide to hold an Inquiry, notice of the date appointed for the Inquiry will be sent you in due course. A print of the National Health Insurance (Medical Benefit) Regulations (England), 1913, is enclosed herewith for your information.
