    PRELIMINARY QUESTIONS. I ft. What is the Name of the Ship, and the Name of the Commander or Mailer ? 2d. Are you the Commander or Mailer ? Where does (he belong to ? 3d. From whence do you come ? 4th. To what Place are you bound ? 5th. At what Ports have you touched iince you left the Port of your Lading on your homeward Voyage ? ^th. What Ships have you had Intercourfe or Communication with on your Paffage, and from whence did they come ? 7th. Did the Plague or any other infedlious Difeafe or Diftemper prevail in any Degree at the Place from whence you failed on your Homeward Voyage, or at any of the Places at which you have touched? if any, fay at which and when? Are any Perfons on board your Ship fuffering under any infedtious Difeafe ; or have any Perfons died or been ill of a Difeafe of that Nature in the Homeward Paffage ; and if any, what Number ? And if any have died or been ill of fuch Difeafe, were their Bedding and Clothes deftroyed ? [If the Ship Jhall have failedfrom any Port in Europe within the Streights of Gibraltar, or on the Continent of America.] 8th. Have you on board any Goods enumerated in this Lift ?
