![29th September 1812. In the courfe of your Voyage have any Perfons on board fuffered from Sicknefs of any Kind; what was the Nature of fuch Sicknefs, and when did it prevail ? How many Perfons were affe6ted by it, and have any of them died in the courfe of the Voyage ? 3«-ft 7uly i8i3* ift. Did you hear of any Report, or are you aware of any Sufpicion having exifted at the Time of your Sailing, that the Plague or any other infeftious Difeafe pre- vailed at the Place from whence you failed, or at any other Place in the Mediter- ranean [or in America or the Weft Indies, as the Cafe may be] ? 2d. At what Port did you take on board your Paffengers ? Were they Refidents at that Place, or had they been embarked on board any other Veffel from any other Places, and from what Places and at what Time ? 3d. Have you any Packages or Parcels which you have taken Charge of; if fo, what are their Contents, and when and where did you take them on board ? 20th September 1813, ift. When did you fail from the Port or Place from whence you took onboard your Outward Cargo, and at what Place did you touch before you arrived at the Port or Place where you took in your prefent Cargo ? 2d.
