In very pronounced rheumatic cases sodium salicylate may be given in doses of 10 or 15 grains every two or three hours until relief is felt. Some apply salicylic acid directly to the surface of the tonsils. The following formula has been suggested:— R Acidi Siilicylici ... ... ... 2 di-aras. Sodii bicarbonutis ... ... ... 1^ „ Glycerini ... ... ... ... 1 oz. Aquae menthis pip. ... ... ad 4 ,, M. A tablcspoonful every 2 oi- 3 hours. An active saline purgative must also be given, such as 2 drams of sulphate of soda or magnesia, with 20 grains of carbonate of magnesia in an ounce of peppermint water every four hours until free action of the bowels'is established. We have seen advantage follow the addition of 5 . minims of ipecacuanha wine to each dose of this aperient mixture, especially when the patient is seen for the first time in the more advanced stage, and when it is desired to hasten sup- puration. The attack can sometimes be arrested at the very commencement by scarification of the inflamed ton.sil, the incisions being allowed to bleed freely. The subsequent application of a 10 per cent, solution of cocaine to the suiface of the tonsil appears to hasten resolution. Or the injection of a few drops of this solution into the inflamed tonsil by means of a hypo- dermic syringe provided with a long needle, in the early stage, is often of great value in relieving pain. When the disease cannot be arrested, cocaine may also be of service in allaying suffering.
