Description

Shows a series of events during a busy operating list which place patients at risk. Not everything is ready for the first surgery and so a decision is made to swop the first and second operation around. Thereafter a catalogue of errors ensue: the wrong patient is delivered to the operating theatre. A man arrives anaesthetised but hasn't had his false teeth removed. He vomits and then moving him into recovery, his IV line falls out. Surgical shots are shown. Finally, the patient falls off the operating table. A member of the production crew steps in with a clapper board and says 'take two'.
