<p>Form published by the UK government in wartime to allow extra supplies of milk and dried milk to those in greatest need of it. The form was to be posted to the local Milk Officer (in this instance at Pewsey, Wiltshire). Revised in October 1941, but has a printing date of August 1941. In this instance the form has been overtyped with humorous extracts from letters received by Milk Officers, taken from Lister's Magazine.</p>
