[here state the Christian Name, Surname, and usual place of Residence of the Candidate] being desirous of admission into the Anthropological Society, I propose and recommend him as a proper person to become a Fellow thereof. day of 18 from personal knowledge. I, the Undersigned, being elected a Fellow of the Anthropological Society, do hereby promise that I will be governed by the Rules of the said Society, as they are now formed, or as they may be hereafter altered or amended: provided, however, that whenever I shall signify, in writing to the Society, that I am desirous of withdrawing my name therefrom, I shaU (after the pay- ment of Annual Contributions which may be due by me at that period, and after giving up any Books, Papers, or other property belonging to the Society, in my possession or entrusted to me,) be free from this obligation.
