There is great atrophy of the tibialis anticus. muscles of the calf are smaller than natural. There ii atrophy of any other muscles of the leg. Knee-jerk abolis Upper extremities.—Great atrophy of thenar and hypoth muscles and probably interossei. All the other arm mm seem natural. Marked fibrillar contractions in arms and le, The general resemblance of this to the preced cases is apparent, but there are these important < ferences, the muscle of the leg first attacked is tibialis anticus; the hand muscles are affected, wh we shall see is a common incident in the later p gress of the disease; there are marked musci motor phenomena, and there is heredity. Case IV.—Edward M., aged 46, a bricklayer, was admi into John Ward, St. Bartholomew's Hospital, in June, II He has had gradual wasting of the right leg 9 years. The is very weak and much colder than the left. There has 1 very little change for the last 3 years, but the difficulty in w ing on it increases slowly. He has occasional cramps in leg. Three weeks before admission he had been suffering f rheumatism, which he said has settled in the affected limb, heredity can bo made out. Has always been temperate. Condition on admission.—Healthy looking man. Chest upper extremities natural. Lower extremities.—Left natural. Right thigh natural, ii leg and foot much wasted. Muscles on outer and anterior as] of leg, more wasted than calf muscles. Tibialis anticus m wasted. Fibrillar contractions occasionally seen.
