COofnty[towitOUgh]} Y°U are re1uired t0 aPPear bef0re {c s j n P S* C I a^ ^e eour* for petty sessions], holden on the day of next, at the hour of in the answer the complaint this day made to me by noon, to that at or in the premises above-mentioned [or in or on certain premises situated at in the Street, in the parish of [or other sufficient de- scription of the premises'], in the district of [describe the S.A.], the following nuis- ance exists [describe it], and that the said nuisance is caused by the act or default of the occupier [or owner, or if the person causing the nuisance be summoned, by you, A.B.] that there existed recently, to wit, on or about the day of y the following nuisance [describe it], and that the said nuisance was caused by the act or default of the occupier [or owner, or by you, A.B.], and although the same has since the said last-mentioned day been abated or discontinued, there is reasonable ground to consider that the same or the like nuisance is likely to recur at the said premises.
