"Thalidomide Points assessment. Correspondence. Individual changes"

Inside the folder is inscribed: "Thalidomide Trust score and reassessment results 1991". Comprises: medical examination reports from 1973 and 1974; guidelines produced by the Thalidomide Trust on medical reassessments, with details from a specific beneficiary (1980 to 1983); correspondence about reassessments from the 1980s; correspondence regarding reassessments of specific individuals (January 1992 to January 1997) and correspondence regarding arrangements for the reassessment meetings (March to June 1991); adapted scoring sheet based on the 1970s assessment form, with various revisions (1991); and on notes on the differences between the 1970s and 1990s assessments.

The file originally included two x-rays. These have been removed and catalogued separately as PP/CNE/A/2/9: "Thalidomide Points assessment. Correspondence. Individual changes": X-rays, for storage and preservation reasons.
